Prior to your first scheduled appointment, please download and complete the Intake and Credit Card Pre Authorization Forms. Please read, sign or initial ALL other forms in the designated areas. There is no need to print all of the pages of the documents requiring only signatures; simply print the signature pages after reading these documents thoroughly. By completing these forms prior to your first appointment, we will be able to use your appointment time more wisely.
